Data required regarding Medical History

The data regarding Medical History of students is mandatory part for all students. Please follow the following instructions:

The proforma is mandatory part for semester registration (Spring-2021). The student failing to submit the Medical Health Proforma will not be able to complete the semester registration.
The last date to submit the online proforma for session 2017, 2018 & 2019 is 05.03.2021.
The students of all sessions are required to complete this form online at https://bit.ly/medicalproformacomplate and Offline.
The proforma must be signed by the student and verified by any recognized Medical Officer.
The students of 2017 session will submit the proforma in hard from in the respective Academic Cell of their departments.
The students of 2018, 2019 & 2020 will send the scanned copy of singed and verified proforma through email to the representative of Academic Cell of their respective departments (department wise academic cell email list attached).
